Application — Everybody Plays

Application for Financial Aid

Financial Aid will not be considered unless this form is completed with a copy of the sports registration form. An application must be filled out for each sport, each year.

Applications from individuals or organizations who apply for Everybody Plays funding will be reviewed and approved by the Everybody Plays Board. Approval will be based on meeting household income requirements, and submission of proper supporting documentation. Once documents have been reviewed and approved, you will receive an email of acceptance and funds will be distributed accordingly.

Who can apply:

A parent or legal guardian of a youth, K-12 who resides between Huber Heights and Springboro, and New Lebanon and Xenia Ohio that meets the income guidelines listed below:
